Fortunately, your collectibles will carry over across multiple play throughs. When you exit the structure, it will be locked behind you. The collectibles in the very beginning of the game before you enter the open world can be missed. There are some collectibles that can be missed.
Luckily, there’s not a whole that can be missed. A feature that has been confirmed to be coming eventually. So if you miss something, you’re going to need to play through again or wait for the game to updated with a mission select feature. There is no chapter or mission select at the time of launch. Unfortunately, there are some missable Achievements.
Everything except the difficulty specific Achievements can be done on any difficulty you want and turning Skulls on does not disable Achievements. You will need to find all of the Skulls before you can activate them for your LASO run. It is going to take multiple play throughs in order to complete.
